OUR CARE / HOME CARE
Support for the everyday.
Practical, personal support that helps you stay comfortable in your own home. The care plan would set out what you want help with, when visits are needed and what you prefer to do yourself.
WHAT SUPPORT CAN LOOK LIKE
Small things. A better day.
The examples below describe the support we are preparing to offer. Your assessment and agreed care plan would specify exactly what can be provided.
- Getting readyHelp with washing, bathing, dressing, grooming and oral care, with privacy and your preferences respected.
- Meals and drinksPreparing simple meals, offering drinks and supporting eating where this is part of the agreed plan.
- Moving around at homeAssistance with everyday movement and transfers using the assessed techniques and equipment for you.
- Medication routinesReminders or agreed assistance with prescribed medicines, within a documented plan and the professional’s competence.
- Everyday companyConversation, familiar activities and encouragement to keep doing the things you enjoy.
- Keeping people informedRecording the support given and sharing concerns with you and the people you have agreed should be involved.
A LITTLE MORE PRACTICAL
How this might fit into a day
A morning visit might include help getting washed and dressed, preparing breakfast and supporting your usual medication routine. An evening visit could focus on a meal and getting comfortable for the night.
Before care begins, we would agree
- Your preferred visit times and daily routines.
- Any mobility aids, equipment or existing support at home.
- Which tasks you want help with and which you want to keep doing independently.
Care depends on assessment, suitability, staffing and availability. Clinical tasks would only be agreed where the required professional skills and arrangements are in place.
There’s room for
