OUR CARE / COMPLEX & NEUROLOGICAL CARE
More complex needs. The same human focus.
Home support for adults whose medical or neurological needs make everyday care more involved. We would first establish exactly what support is needed, which professionals are involved and whether we can safely meet those needs.
WHAT SUPPORT CAN LOOK LIKE
Understand the detail. See the person.
The examples below describe the support we are preparing to offer. Your assessment and agreed care plan would specify exactly what can be provided.
- An individual care planBring together your daily support needs, communication preferences and guidance from the clinicians already involved in your care.
- Personal care with additional needsPlan assistance with washing, dressing and toileting around symptoms such as weakness, fatigue or reduced movement.
- Movement and positioningFollow an assessed moving-and-handling plan, using appropriate equipment and agreed positioning support.
- Communication supportAllow time and use your preferred communication methods, including aids or approaches recommended by your clinical team.
- Agreed clinical supportDiscuss medication assistance, monitoring and any specific clinical tasks individually. Each task requires an appropriate care plan and a professional with the right training and competence.
- Coordinated careRecord changes or concerns and follow the agreed escalation plan with your existing GP, community team or other clinicians.
A LITTLE MORE PRACTICAL
How this might fit into a day
For someone living with the effects of a stroke, a visit could combine personal care, assistance with an assessed transfer, time to communicate and support with routines agreed with their wider clinical team.
Before care begins, we would agree
- The exact clinical tasks required, rather than the diagnosis alone.
- Existing care plans, specialist advice and necessary equipment, shared through an appropriate process.
- Whether the required skills, supervision and staffing can be put in place before care begins.
Care depends on assessment, suitability, staffing and availability. Clinical tasks would only be agreed where the required professional skills and arrangements are in place.
